Head of Department Nursing/Team Lead Coordination Spitex (80 -100%) / Permanent Introduction On direct mandate of our client, a sympathetic, renowned and strongly growing Spitex organization (PA-model) with a sympathetic corporate culture (work location Stadt Bern or Stadt Zürich selectable, central locations, parking spaces in front of the building, regular weekly working hours), we are looking for the expansion of the team for an experienced Dipl. Pflegefachperson / BSc Pflege for the department and team leadership as well as occasional case management (without nursing activities). Nature of work In this versatile function, you will lead a collegial team of specialist professionals (Dipl. Pflegefachpersonen and Dipl. Fachpersonen Gesundheitswesen), which takes over the case management as well as care planning for the care including preventive support of clients at home. You plan the nursing services cross-functionally including supervision during needs assessments, goal setting and deployment planning for the Dipl. PH/FaGe's and nursing relatives (PA), who care for the patients on site. You provide occasional support in case management and coordination with health insurance companies, stand by the teams in an advisory capacity and support in the consultation of clients (by telephone, occasionally also on site - e.g. during status assessments or complex topics). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative personality, you also find the active cooperation and exchange of information with various contact persons (management, care team, external bodies such as health insurance companies or authorities) easy. Requirement profile To be successful in this exciting job, you have completed the dipl. Pflegefachperson HF/FH or an equivalent further education and have possibly additionally undergone further training (e.g. NDS Medical or BWL). In addition, you have already been able to gain versatile practical experience in leadership, case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well organized and possess great ability to introduce/train others as well as a healthy dose of humor.
