Responsibility/ Main tasks: Independent, competent and resource-oriented care of our clients in the home environment; Direct exchange with our customers, relatives and the interdisciplinary team; Digital documentation for efficient work organization with Perigon; Assumption of expanded responsibility and co-design of modern nursing processes, case management; Day responsibility in intervals. Qualification/ Requirements: Completed training as Pflegefachperson HF (SRK recognition for foreign diplomas); Inter Rai HC knowledge is an advantage or willingness to complete the basic course; Proficient in the application and execution of the entire nursing process including needs assessment; Joy and motivation for nursing in the outpatient sector; High social competence, teamwork and empathy; Very good German skills in speaking and writing at least B2; Driver's license Cat. B and own car; Structured and independent way of working.
