Pflegefachperson (HF/FH) (m/w/d), 50-80 % Your role • You advise and accompany caring relatives as well as people in need of care in their home - empathetically, professionally and solution-oriented • You conduct initial meetings with our clients on site and take over the needs assessment and care assessment • You ensure a professional, structured and trustworthy first impression with our clients and relatives • You analyze care situations holistically, make differentiated assessments and initiate corresponding measures • You create and are responsible for care planning and the correct preparation of the care prescription • You ensure seamless, structured documentation and guarantee a clean handover to internal interfaces • You coordinate cooperation with doctors, health insurance companies and other partners • You take responsibility for the quality, further development and documentation of care processes • You ensure efficient work organization and actively contribute to the development and optimization of our structures • You make a personal contribution to a positive, cooperative team climate • You show willingness for continuous professional and personal development What you bring • Completed nursing training at the tertiary level (Dipl.
