Our team of around 85 employees brings a lot of commitment to the communities of Bichelsee-Balterswil, Fischingen, Eschlikon, Münchwilen, Rickenbach, Sirnach and Wängi, bringing joy to the homes of our clients. A meaningful task that is fun. We are looking for a Pflegefachfrau / Pflegefachmann Bedarfsabklärung with a workload of 80-100% immediately or by agreement. Your tasks: Bedarfsabklärung: Conducting needs assessments with clients at home to determine the care needs and required services. InterRAI HC-Assessments: Applying the InterRAI HC system to conduct standardized needs assessments, ensuring precise care and support planning. Consulting: Consulting clients and their relatives regarding care and support services. Creating care plans: Developing individual care and support plans in cooperation with care professionals and the interdisciplinary team. Coordination: Close collaboration with care staff, doctors, Spitex employees, and other specialist offices to ensure seamless care. Documentation: Good knowledge of creating care plans and needs assessments. Strong communication skills and empathy in dealing with clients and their relatives. Independent, structured, and responsible work. Careful and timely documentation of needs assessments and care planning in the system, particularly InterRAI HC data. Quality control: Contributing to ensuring high-quality standards in care and implementing further developments in the field of needs assessment. Close collaboration with the Stv. Pflege and Betreuung/ Pflegeexperte/in Your profile: Diplomate Pflegefachfrau/Pflegefachmann HF or FH (or comparable training in the healthcare sector). RAI expert, InterRAI HC experience in Spitex: Extensive experience in caring for and supporting clients in their home environment, particularly in Spitex. Knowledge: Experience in applying the InterRAI HC system for needs assessment and care planning, or willingness to learn this system. Interest and ability to conduct needs assessments independently and in a solution-oriented manner. Very good communication skills and empathy in dealing with clients and their relatives. Independent, responsible work, as well as high flexibility and resilience. Very good IT skills and experience with documentation systems.
