ASPMAD, IT IS: - 9 CMS in the North of Vaud - More than 1000 collaborators engaged - More than 6'700 people accompanied each year - A presence in 105 communes Nurse referent in mental health Yverdon FOR OUR CMS OF YVERDON-OUEST, WE ARE RECRUITING A: NURSE REFERENT IN MENTAL HEALTH AT 80% YOUR MISSION As a mental health nurse, your role as a resource person for your future team implies an advanced mastery of specific skills: Clinical evaluation: Carry out regular evaluations to adapt care to the evolving and complex needs of clients. Interprofessional collaboration: Work in synergy with a multidisciplinary care network to optimize care. Leadership and mentorship: Provide guidance and support to your colleagues, thanks to your role as a resource person in mental health, acting as a reference within your team. Planning, coordination and management of documentation: Organize personalized care plans in partnership with clients while keeping client records up to date in accordance with protocols, including OPAS sheets. YOUR PROFILE You hold a HES diploma in nursing or a recognized equivalent title, completed by at least two years of experience in adult psychiatry and/or geriatrics. A post-graduate diploma in mental health would be a plus. Endowed with autonomy, versatility and a great ability to listen, you have a marked interest in maintaining at home and collaborative work with different disciplines. You have the ability to adapt easily to various situations, and you excel in communication and negotiation. You have a driver's license and, ideally, a personal vehicle. WHY JOIN US?
