COORDINATION NURSE For our coordination service based in Estavayer-le-Lac: Main missions Ensure the coordination, transition and continuity of care for clients, guests, residents in the district of the Fribourg Broye. Evaluate the needs of people with loss of autonomy at home or after a hospital stay (HIB, HFR) Analyze requests for aid and care services. Implement and coordinate necessary services within a period of 8 hours. Collaborate closely with treating physicians, hospital teams, as well as social and health institutions in the network. Contribute to the fluidity of care pathways and the optimization of network health resources. Actively participate in institutional projects. Desired profile HES graduated nurse or equivalent training. Experience of 5 years in home care or care coordination. CAS in care coordination or willingness to train. Knowledge of the care network and the Swiss health system. Excellent analytical, organizational and communication skills. Ease in interdisciplinary and network work. Mastery of common computer tools (computerized patient file, request management). Driving license (travel in the Broye).
