Depending on your profile, your mission will consist of: • Evaluating functional capacities and needs at the beneficiaries' homes • Developing and implementing an intervention plan promoting autonomy in activities of daily living • Analyzing and adapting the living environment in order to prevent risks and secure the home • Collaborating closely with the interdisciplinary team and network partners • Participating in prevention and health promotion actions (fall prevention, support for family caregivers, etc.) • Ensuring the traceability of interventions and the writing of reports intended for partners and insurance Locations CMS de Rennaz and Montreux Profile • Bachelor's in occupational therapy (HES level or equivalent); recognition from the Swiss Red Cross or SEFRI required or in progress for foreign diplomas • Experience in rehabilitation and/or geriatrics, an asset • Marked interest in home care and in supporting elderly people as well as people affected in their mental health • Good capacity for analysis and evaluation of home living conditions in order to adapt interventions and promote the autonomy of beneficiaries • Excellent interpersonal skills: listening, communication and negotiation skills • Sense of responsibility and solid professional ethics • Ability to work autonomously while collaborating within a multidisciplinary team • You share our values of transparency, equity, respect and trust in all your professional interactions • Mastery of IT tools (patient file, planning, quality monitoring) • Driver's license indispensable.
