Liaison Nurse / at 80% / in CDI

The specific field of the liaison nurse covers hospitalized imad clientele (known or future clientele) requiring a return home with imad services. Their field of activity includes, in particular, the assessment of needs, medical validation of required services, and the predictive coordination of interventions by the home maintenance teams (EMD) and/or centralized services.

Your missions and responsibilities

• Inform clients or future clients of the home services offered by imad, the partners of the Geneva socio-sanitary network, the rates and procedures for reimbursement of home maintenance services. As necessary, direct the client to the person or structure that can advise them.
• Ensure coordination between imad and the private/public hospital sector to organize the return home of clients, with services provided by imad.
• Promote the exchange of useful and necessary information for the client's care (according to the planned documentation).
• Facilitate clients' hospital discharges and promote a return home under the best possible conditions.
• Collaborate with the client, the caregiver, and health and social professionals.
• Prevent inappropriate (re)hospitalizations.
• Work in close collaboration with the imad arcade team located at the HUG.
• Bachelor HES-SO in nursing or training deemed equivalent and registered by the Swiss Red Cross
• Knowledge of the care process and clinical pathways, and the Geneva social health network
• Ability to work alone and in interprofessionality within a multidisciplinary team
• Sense of responsibility and solution orientation, sense of communication, negotiation, and relationship with others
• Ability to adapt and delegate
• Stress and conflict management
• Resistance to physical work.
