Your tasks: Responsible care, support and advice for people of all age groups in their familiar environment; Case management including performing interRAI-HC assessments and controlling the nursing process (not a requirement); As the daily responsible person, you are the contact person for interdisciplinary partners; you decide, organize and delegate arising and additional tasks; you support employees professionally. Your profile: Diploma as Pflegefachperson HF, DN II or AKP; Joy in taking on responsibility and making decisions; Independent, solution-oriented way of working; High professional, social and communication competence; Team spirit, flexibility and joy in innovation; Joy in using digital working tools; Driver's license Cat.
